A Better Life Bradford Fact Sheet

What does it do?
A Better Life is the free adult Weight Management service, commissioned by Bradford Council and delivered by ABL Health, in partnership with the Thornbury Centre and HALE (Healthy Action Location Engagement). The service is delivered by a team of experienced and qualified Lifestyle Coaches who are supported by a multidisciplinary team which includes a GP, psychologist, nutritionist and exercise specialist.

Who is it for?
Adults aged >18 years Living within the Bradford council boundary BMI 25 to 35 (or up to 40 with no co-morbidities)

Referral through a GP, health professional Self-referral by phoning 01274 299022

What can clients expect from the programme?

  • Personalised assessment; incorporating expectations, challenges and aspirations
  • 12 weekly group sessions, including nutrition, exercise and behaviour change
  • For adults that require additional support 6 x 1 hour bi -weekly sessions can be offered
  • Up to 10 months of monthly 'Check in and Chat' group support
  • Evidence based behaviour change APP support
  • Signposting and connections to local community networks
  • Face to Face reviews at 6 and 12 months to assess progress and sustainable change

 

What further information may i need?

ABL are able to provide tailored support to groups, i.e clients with learning disabilities. These groups will be delivered in partnership with local services.
