Leaving Social Care

What does it do?
Provides Social Work support for eligible and relevant young people leaving care and for those young people deemed 'in need' as a consequence of homelessness, which can include working with partner agencies (housing and accommodation providers), signposting to appropriate services, carrying out assessments of need, individual support towards independent living and their transition to adulthood, limited support to assist young people to return home (if an appropriate option) through mediation and possible financial support to those young people deemed eligible, relevant, and/or in-need within the policies and procedures issued as guidance to the Leaving Care Service. The team is made up of Social Workers and Community Resource Workers and is part of the Bradford’s Social Care Services for Children and Young People; the team also have a network of partner agencies, in-house volunteers and training & employment resources.

Who is it for?
The Leaving Care Service provides support and assistance to eligible and relevant young people leaving care and for those young people deemed 'in need' as a consequence of homelessness.

How do I access it?
Through Children's Social Care; young people leaving care will already be known to the service and referrals will have been made as part of the transitions process. Homeless young people assessed as 'in need' will access the service via housing and homelessness routes.
