Nursing Team for Children Looked After (CLA), Youth Offending Team (YOT) and Leaving Care (LC)

The Childrens Looked After Nursing Service 

All looked after children and young people living within the Bradford Metropolitan District Council (BMDC) aged 0 to 18 years of age have access to the Childrens Looked After (CLA) Nursing Service.  

We are a team of specialist nurses, nursery nurses and administrative staff located at Westbourne Green Health Centre, Bradford.  Each looked after child and young person has their own named nurse who remains with them during their journey through the care system. 

What we do:

The specialist nurses ensure that the looked after children and young people in Bradford are offered an individualised, age-appropriate six monthly or annual health assessment as per statutory guidance which supports and promotes the health and wellbeing of this cohort. We also offer these assessments at a time and venue convenient to the child and young person. 

Our nursery nurses can support with a range of identified health issues including child development, behaviour management, continence, and sleep. 

We liaise with a wide range of other services and professionals both within health and other agencies.

Youth Offending and Leaving Care Nurses

Bradford Youth Offending Team Nurses work within Bradford YOT and provide health assessments, support, advice and sign-posting to all young people aged 10 – 18 and under (and their carers)  who are involved with the juvenile criminal justice service throughout the Bradford District.

The Leaving Care Nurses work closely with social care services to promote the health needs of young people aged 18-25 who are leaving the care of the local authority. This includes supporting young people moving towards independent living.

Our service includes:
•    Providing health advice, information and support
•    Undertaking health assessments and care planning
•    Providing consistent care with a named Leaving Care nurse
•    Referring to specialist services

All looked after children and young people are allocated their own CLA nurse even if the child or young person has come from outside the BMDC area.  If you would like to get in touch about any looked after child or young person, or need any advice, please contact the Childrens Looked After Nursing Service on Tel. 01274 221002.
