Year 5, Year 10 Transition Planning

During summer term 2022, the majority of schools will be holding a transition review for children’s moves to either primary or secondary school or to a post 16 college or provider. The Integrated Assessment Team has asked schools in April 2022 to complete all transition reviews by 22nd July 2022. This is so the Local Authority can amend your child’s EHC plan, and begin to request consultations with a families choice of school(s) as well as a local authority preference for September 2023.

The Integrated Assessment Team can confirm that all families who have children or young people have been written to requesting their choice. All schools were also provided with a copy of this letter in April 2022.

The below timeframes have been provided to our families so we can best support them in their child / young person’s phase transfer.

Please could you provide your preference/s for your child’s potential next school or post 16 college or provider by:

  • Friday 16th September 2022 – request for mainstream provisions
  • Friday 30th September 2022 – requests for Specialist Provision (including Resourced Provisions)

At the end of the annual review paperwork a families preference(s) can be recorded in the ‘Actions’ box. Or, they can be emailed to us separately - at transitionreviews@bradford.gov.uk  using the following codes in the body of your email response:

  • PP – name of setting
  • PP – name of setting

If families have more than 1 preference then please continue the naming structure (PP, PP etc.) in your email. Please note this is not a ranking system but a way of organising families thoughts / preferences.

We understand that some families may not have the facilities to email, therefore will still accept preferences by post to the address provided in the letter. We would ask schools to continue to support families and may email our service on behalf of the family with their preference(s) if they so wish.
