A multidisciplinary specialist service for Adults aged over 16
patients with rehabilitation needs are assessed by the team including doctors, physiotherapists, occupational therapists and specialist nurses.
Transition is managed jointly across teams for young people moving from paediatric to adult services to ensure needs are met by most appropriate team
What does it do?
The Specialist Rehabilitation Team offers expert assessment and advice to adults with long term neurological conditions. The team is led by a consultant in rehabilitation medicine. To address areas of concern regarding; Your Health and medication, Day to day activities, Work and leisure.
Who is it for?
Patients over the age of 16 living in the Airedale NHS Foundation Trust catchment area
Your doctor, specialist nurse, consultant or other healthcare professional can refer you.
Transition pathways are in place for young people aged 14 + to facilitate smooth handover from paediatric services
